My wife and I are trying to have a baby, but it’s not going so good. She had a spontaneous abortion about 18 months ago. After that we waited about six months and began to try again. We got no result in the past year. My wife is going crazy. Can you give us some advice how to determine the fertile time? She is measuring her temperature and we are trying after the temperature is higher with 0.4, but nothing for the past three months. Â Is there something more we can do?

The temperature rise of .4 degrees that you refer to usually means you have already ovulated. And since the life span of the egg is very brief once it is released (12-24 hours) you are missing the window of fertility by waiting for that temperature rise! Some women experience a dip right before the rise. That would be a better indicator of fertility than the rise, if your wife observes it.
You ask if there is anything else you can do (to time relations to achieve a pregnancy). Yes, I would recommend your wife learn to observe the discharge of cervical mucus, the best indicator for timing relations to achieve a pregnancy. The most reliable way to learn how to do this, is to consult an expert who can teach you how to observe, chart, and interpret the signs of fertility her body produces. While you pursue that, in addition to taking your temperature as you have been, I would advise your wife begin to pay attention to the discharge of cervical mucus produced by her body, as she wipes for normal hygiene on each trip to the bathroom. The observation of a mucus discharge on the tissue, especially if accompanied by a lubricative sensation, and exhibiting clearness and stretchiness, are signs closely correlated with ovulation, and indicate a good to day to achieve pregnancy. These signs will precede the temperature rise.
